Metrics and reading notes

Periods are kept separate. Annual or pooled data is never divided into quarters. Missing slices remain unavailable. A CSV contains every metric for the selected rows and periods, including sources and definitions.

How to read this table

Annual application counts. Emergency services and surprise bills are separate series. Split decisions can span multiple service codes.

  • These are captured historical state reports, not live filing counts or CPT-level results. Reporting units differ between programs and sometimes between years. Do not pool them with federal line-item data.
  • Provider win % = provider wins ÷ (provider wins + payer wins). Split decisions and other outcomes are excluded. Reported defaults remain included. No rate is calculated if either party count is missing or their sum is zero.
  • Decisions, party wins, closures, and filings may have different denominators. Party wins do not always sum to the decision count. Read each row’s report note before interpreting differences.
  • Award columns, where available, show aggregate dollars reported by the program. They are not means, medians, or expected recoveries. All-empty metrics are omitted for that state.
  • CSV downloads include the filtered rows, every reported metric, exact reporting periods, definitions, notes, and source URLs. Column selection only changes the screen view.
