Nevada is a bifurcated state. State dispute rules may apply to qualifying services under Nevada-regulated plans and self-funded plans that opt in.
Plan and jurisdiction considerations
Verify coverage type, state jurisdiction, service scope, and any self-funded plan election. Do not infer opt-in status from the administrator's name.
Federal fallback
A self-funded plan without a Nevada opt-in generally remains in Federal IDR for eligible claims, as do claims outside the state payment method.

Nevada's good-faith negotiation, arbitration, filing, and fee provisions are distinct. Confirm current Division materials before acting.
