Maine is a bifurcated state. State rules may apply to qualifying claims under Maine-regulated plans and self-funded plans that elect state participation.
Plan and jurisdiction considerations
Establish whether the plan is fully insured or self-funded and, if self-funded, whether it opted into Maine law. Then test the service against state scope.
Federal fallback
Federal IDR generally applies to eligible self-funded claims without a Maine opt-in and to claims outside a state payment method.
Timing and fees
Use current Bureau forms and instructions for state disputes. State deadlines and costs can differ from the federal 30-business-day negotiation and 4-business-day initiation structure.
