Colorado is a bifurcated state. State law may determine payment for specified services involving Colorado-regulated coverage; other federally protected claims may use Federal IDR.

Plan and jurisdiction considerations

Verify plan regulation, service category, facility status, and whether the Colorado payment framework applies. ERISA self-funded plans generally remain federal unless a valid state mechanism applies.

Federal fallback

Route otherwise eligible claims to Federal IDR when Colorado law does not set the out-of-network amount.
