A submission should identify the qualified item or service, the offer, the QPA information received, and any additional permitted information that is credible and tied to the claim.
Avoid prohibited or duplicative information
Current rules limit what an entity may consider. Verify the governing regulation and guidance, avoid double-counting a factor already reflected in the QPA, and do not assume billed charges control.
