These results describe services that reached federal IDR determinations, using the historical disputed-service mix. They do not describe all out-of-network services or forecast collections. RecoveryIDR analysis of CMS public-use data.
Already outsource billing? A review helps establish whether eligible disputes are already being pursued by your billing team.
Your selected comparison and current entered scenario will be included with your request. Do not include patient information or claim details.
These comparisons use public data from the Centers for Medicare & Medicaid Services (CMS) for federal independent dispute resolution decisions made from October through December 2025. CMS released this data on July 22, 2026. The results cover services across the country that reached a decision, not all out-of-network care.
The awarded amount is the total payment selected in the dispute. The median is the amount in the middle when the results are ordered from lowest to highest. The middle 50% range shows where the central half of the results fell. To find the additional-payment gap, we subtract the insurer’s initial payment and the patient’s share from each award, then find the middle amount and range of those differences.
We include decisions won by either side, including cases where one side did not submit an offer or pay the required fee. We keep zero payments and negative payment gaps. We count a bundled payment once, without counting its individual parts again. Missing or withheld amounts are left out of the calculations; they are not treated as zero.
We show an award comparison only when there are at least 100 usable awards and the services can be clearly identified. A historical payment gap also needs at least 100 records containing the award, initial payment and patient share. This minimum does not guarantee that the results will represent your practice. Categories follow CMS service groupings, may overlap and should not be added together. If a selected service lacks enough information, we say so rather than show a broader result in its place.
Bar lengths are proportional to the largest selected median award, with a minimum width for readability. Small awards at the minimum width are not to scale.
The calculated payment reference is the displayed median award minus the median historical additional-payment gap. It is derived from those two values, rather than an observed median payment. If that reference falls outside the award bar, we show the exact figures in words instead of placing a marker inside the bar.
If you enter a monthly billed-service count, we multiply each service’s historical additional-payment gap by that count, then multiply by 12 for the yearly amount. Each row assumes that every entered service is that type of service. These are alternative scenarios and must not be added together. This assumes comparable eligible services reach a dispute decision. It does not account for fees or predict what you will collect. Visits and operations can contain several billed services, and differences in complexity, treatment time and care setting can affect how useful the comparison is.
